I haven’t finished reading this yet but I began to wonder as I read if you suffered from PCOS. I have 2 family members who do and they have been through similar situations.

My niece who is only 23 years old was morbidly obese from age 2 years on. She never knew what it was like to not be over weight. She suffered from most of what you described, including PCOS. She did have the surgery (the sleeve) and has lost 150 lbs. She feels MUCH better physically and it sure has helped her emotionally as well since she’s so young. It was not an easy decision for her either but I can truly empathize after reading what I’ve read so far.
